For a married woman who has done a change of name, she has to also do a change of name on NIN with NIMC. It is the new NIMC Slip that would be provided. She will also provide the Avidavit Change of Name and Newspaper publication.
From the guarantor side, passport data page, NIN slip and one passport photo.
Note that you will need to download the guarantor’s form and take it to court for stamping after your guarantor has filled the form. You will take the filled and stamped guarantor’s form with you too.
Also note that you should take both original and photocopy of all documents along.
